The end that comes off the spool should be threaded. The end that is cut to the desired length should be knotted.
Some people (especially those who need reading glasses) thread several needles at the same time, leaving them on the spool. When they need a new thread they only have to hold onto one needle and keep the rest of the needles near the spool while they are unrolling the thread. That freshly cut end is then knotted.
